Social Prescribing Link Worker – Newcastle Central Primary Care Network

Permanent

Full time (37 hours a week)

£28,200 per annum

Main base The Hub, Brampton Business Centre, Queen Street, Newcastle-under-Lyme with work in GP practices, some travel and partial home working

This post will work across the Newcastle GP practices of Miller Street Surgery; Milehouse Medical Practice; Wolstanton Medical Centre; Higherland Surgery; The Village Surgery; and Lyme Valley Practice.

You will benefit from experience of supporting people through information, advice and guidance; managing a caseload; and using IT to keep good records.
