22 hours a week over 3 days.
£29,200 pro rata (inclusive of travel costs)
Main base-Stafford based GP surgeries and partial home working.
Join an established and friendly Social Prescribing team based within GP practices in Stafford Central and Stafford South Primary Care Networks.
Do you have great listening and communication skills?
Have you got experience of helping others and a passion for supporting people through information, advice and guidance?
If you have good local knowledge of community services and the initiative to find out more, then being a Social Prescribing Link Worker could be a great new role for you.
You will also need to be organised, able to manage a high caseload in a fast-paced service, have sound IT skills and be able to keep accurate records
However, we are more interested in your passion and energy to make a difference, as full training will be provided.
